Report Form Instructions:
This form is intended for the reporting of major architectural barriers that limits equal access to public facilities and businesses for citizens suffering from disabilities, in accordance with the Americans with Disabilities Act of 1990 (ADA) under Title III. Yet because of the complexity of the ADA, and the specificity of some of the stringent requirements as they may conflict with various local state and city laws, we can only accept submissions of obvious architectural barriers. Please view our Reporting Guidelines before making a submission.

Title II submissions will also be accepted on a case-by-case basis.

At least one piece of photographic evidence is necessary to submit a report. We highly recommend that the owner's name or the property manager's name is also collected to facilitate the process. After reviewing a report, Barrier Watch will send a certified notice to the location owner and attempt to engage in a constructive process towards ADA compliance and barrier removal. You will be kept up-to-date on the status of your submission, but Barrier Watch will not publish the identities and locations of reported sites until at least thirty days of notice have been given.

Note: because of the complex requirements of the ADA and various local state and city laws, we also urge business owners to hire an ADA consultant to preform due diligence on their site before engaging in any construction or renovation.

Terms and Conditions: By submitting this information to Barrier Watch, you agree to indemnify and hold harmless Barrier Watch, any of its associates, employees or affiliates from actions that may occur from its use. Submitted photographs and other evidence will also become the property of Barrier Watch. In turn, Barrier Watch will not disclose your identity during correspondence with the reported party. Furthermore, Barrier Watch will never disclose your personal information to a third party without your express and written permission, unless required to do so by law. You also agree to refrain from publicly exposing the identity and location of the reported site in question until the location's confidentiality period, including any extensions, has expired, at which time Barrier Watch will publish the reported site on its website and contact you, the reporter, upon publishing.

Furthermore, you also agree to refrain from litigation with the reported party involving either the federal ADA guidelines or your state's local guidelines, unless at least one of these conditions has been met: 1) The reported location's confidentiality period, including extensions, has expired, or 2) A separate incident involving intentional discrimination and/or other violation of your civil rights occurred after the report was submitted. Finally, you also agree that Barrier Watch may refrain from or amend its procedure for ADA Title II reports, which involve a much more complex resolution process.
